(Dependent registration is for spouse and child dependents above 18 years of age.)

Note: You can choose to allow access to your health information by other members in your family after registration is complete. After logging into HealthView, this feature can be found under the Member Profile tab under Account Settings.

HealthView is dedicated to protecting your personal health information. To help ensure your privacy, we have implemented an on-line process to verify your identity. A confirmation of your registration and your password will be sent automatically to your email address upon completion of registration.
